Location of problem - Please provide the exact address. If the property is a duplex, both addresses are needed, even if a problem exists in only one unit. If it is a corner, then the exact corner is necessary. In the case of vacant lots, provide the address on either side of the lot. If the request involves an apartment complex or similar development, please provide the name of the complex. General descriptions, such as "behind the grocery store", are not acceptable.

Nature of problem - Please be as specific as possible! Helpful information includes descriptions of the problem (types of junk/rubbish present), vehicle descriptions and license plates, how long the problem has been ongoing, etc.
